Measurement of the tibial anterior-posterior translation for Lachman test in the awake state and under anesthesia using electromagnetic measurement system.

چکیده

Introduction: Anterior cruciate ligament (ACL) deficiency causes an increase in anterior tibial translation. To assess the amount of the anterior-posterior tibial translation, KT-arthorometer has been widely used. However it has been suggested by some reports that the practical reliability of the KT-1000 arthrometer measurement is doubted due to the interand intra-examiner errors. The Lachman test has been recognized as one of the most reliable clinical test in diagnosis for the anterior cruiciate ligament deficiency. Previously, we have developed an electromagnetic measurement system (EMS) to assess anterior knee laxity during Lachman test and reported that EMS provided more accurate measurement for determining the bony movement and had better correlation to the fluoroscopic measurement than KT-1000 arthrometer. However the study was conducted under anesthesia and it still remains to be tested whether the anterior knee laxity during the Lachman test can be measured accurately when the subjects were awake for daily practice. In this study, we assessed the anterior knee laxity using the EMS measurement system and compared the difference between under awake and anesthesia conditions.

Differences in Knee Kinematics Between Awake and Anesthetized Patients During the Lachman and Pivot-Shift Tests for Anterior Cruciate Ligament Deficiency

BACKGROUND The Lachman and pivot-shift tests have been widely used for detecting anterior cruciate ligament (ACL) deficiency. However, it still remains unclear whether these manual tests can be quantified accurately while patients are awake. PURPOSE To assess the differences in knee kinematics of awake and anesthetized patients. STUDY DESIGN Case series; Level of evidence, 4. METHODS A to...

Accuracy of Lachman and Anterior Drawer Tests for Anterior Cruciate Ligament Injuries

Background: The knee joint is prone to injury because of its complexity and weight-bearing function. Anterior cruciate ligament (ACL) ruptures happen in young and physically active population and can result in instability, meniscal tears, and articular cartilage damage. The aim of this study is to evaluate the accuracy of Lachman and anterior drawer test in ACL injury in compare with arthrosc...

Measurement of Posterior Tibial Slope Using Magnetic Resonance Imaging

Background: Posterior tibial slope (PTS) is an important factor in the knee joint biomechanics and one of the bone features, which leads to knee joint stability. Posterior tibial slope affects flexion gap, knee joint stability and posterior femoral rollback that are related to wide range of knee motion. During high tibial osteotomy and total knee arthroplasty (TKA) surgery, proper retaining the...

Fluoroscopic analysis of anterior tibial translation during eccentric and concentric phase of knee rehabilitation exercises in men with anterior cruciate ligament injury

Background: The amount of anterior tibial translation during rehabilitation exercises is a key factor in organizing exercise regimen after anterior cruciate ligament injury. Excessive anterior tibial translation could increase the magnitude of tension imposed on injured and reconstructed anterior cruciate ligament knees. Forward lunge and open-kinetic knee extension exercises are commonly used ...

Biomechanical analysis of the knee with partial anterior cruciate ligament disruption: quantitative evaluation using an electromagnetic measurement system.

PURPOSE To investigate the biomechanical function of anterior cruciate ligament (ACL) remnants in ACL-deficient knees with both partial and complete tears. METHODS Twenty partial ACL-deficient (group P), 20 complete ACL-deficient (group C), and 40 contralateral ACL-intact knees were examined. The end point during the Lachman test, side-to-side differences of KT-1000 measurements, and the pivo...
